DS-11 Form
Form DS-11 is required for all first-time applicants. It is also necessary for those seeking to renew a previously issued passport. It is also needed for those seeking a replacement due to damage, such as water damage or unofficial markings on the data page.
Form DS-11 is used to apply for a passport book or card at an acceptance facility. This form also renews or replaces a lost or stolen passport. It must be completed and submitted in person before a passport agent at an official Fogarty International Center (CIC) Passport Agency. If the applicant is under age 16, a parent or legal guardian must appear with the applicant and sign Form DS-11. The parent or guardian must also provide a photocopy of their ID. The applicant must have the form completed in black ink, or it will be rejected. If the form is filled out in another color, the applicant must complete a new application. How Long Will My New Passport Take to Process?
A new passport will usually take between 4 and 6 weeks to process. However, you can apply for expedited processing if you have an urgent travel plan. You must submit supporting documents and pay an additional fee to do this. The processing time is subject to change throughout the year, so be sure to check the website for updated information.
It is important to remember that DS-11 is used for passports for people under the age of 16. Children who are not legally adults must apply in person with parents or guardians. In addition, the form must be signed by both parties. If the parents are unable to attend the appointment, they must complete Form DS-5525, “Statement of Exigent/Special Family Circumstances”.
