Form 7004 – Online Fillable PDF

Form 7004 is your Application for Automatic Extension of Time To File Certain Business Income Tax, Information, and Other Returns.

This tax form shouldn’t be mistaken with Form 4868, which is for individual income tax returns. Form 7004 is only for business tax returns that are listed on the form’s Part 1. It’s basically the tax form you use for requesting an extension, but not for individual returns.

Each return listed on form 7004 has a corresponding code. Enter it on Line 1 of Part 1. You can only enter the code once per form, so you will have to file multiple copies of Form 7004 to request an extension for more than one tax form.
